This is the 2nd time in 2 weeks now I’ve lost one of my chaos dungeon entries due to this error as well as I lost rested bonus. Is there any way to get one of those potions to retry for my chaos dungeon?

I am sorry to hear about the lost chaos dungeon, currently only the automated system could provide a re-entry as we do not have an option to issue items or entry tickets, this can take up to 24 hours at max from the time of the crash and it will delivered to your in-game mail.
We can’t guarantee that you will be receiving a ticket, I apologize for the inconvenience. Here’s a post that explains better how this system works.
