Mayhem; No Health Refund After Zone Transition

When deactivating Mayhem, you are refunded HP back. However, sometimes when leaving an instanced area or tri-porting, the game deactivates Mayhem for you, but does not refund the HP- leaving you with an annoying red border around your screen due to low HP.

Not sure if this is a bug or intended.

You can turn off the red border and the sound when on low health in the game settings. I was super annoyed by that, too.
