If you’re referring to the T3 Chestpiece from the Chaos Dungeon, obtaining it is just a random chance. It takes some people a few tries to get all the armor pieces required for their first complete T3 Armor set. I was missing my sword for a few days myself.
If that’s not what you were referring to, could you please provide more details on what T3 Chest you’re referring to?

In this case the chances to get the T3 chest are random, some players can get it on their first try others will have to try again, this is normal behavior don’t worry, you’ll get it.
